Denton Review Template Introduction
On April 6, 2020, notice was sent to the organization that merit increases would be frozen
effective April 18, 2020, through at least the end of the fiscal year and potentially longer. As
was noted in the communication, performance discussions would continue, but associated pay
increases would not be included.
To assist supervisors and employees in having these important performance discussions in a
timely and meaningful manner, Human Resources (HR) has created a new short-form review.
This new form will replace the existing “Performance Appraisal Template” and will be used for
all performance reviews going forward. This new short form has also simplified the review
process by making the employee self-review optional, as well as removing the multiple “CAM
model” competencies, the strong-minus/strong-plus rating system, the IDP, and the need for a
status sheet to be completed along with the review.
New Performance Review Process (Effective April 18):
• Performance reviews will still be completed on the employee’s anniversary date.
o HR will continue to notify supervisors via email when the employee reviews are
due.
• The new review form contains 15 questions relating to the employee’s overall
performance and approach to work.
o The supervisor will answer each question about the employee being reviewed.
o If desired, the supervisor may seek feedback from customers and coworkers to
obtain a more accurate picture of the employee’s performance.
o While the employee self-review is no longer required, if the employee would like
to complete a copy of the questionnaire in order to be able to discuss
performance with the supervisor, they may do so. If the employee does
complete the form, they should email it to their supervisor prior to the
performance review meeting.
• The second page of the form contains space for both the employee and supervisor to
provide specific written feedback.
o The objective of this section is for managers to set clear expectations for the
employee, highlight praise or detail concern about performance, and outline
development opportunities.
o Whether the employees fill out the questionnaire or not, the employee should
use this section to communicate their levels of satisfaction, engagement, and/or
concern, address known performance issues, as well as express to their
supervisor how they can be supported and developed going forward.
o This section will also allow both parties to set business and personal goals.
• We are still asking that the review be completed manually for the time being. This new
document is a fillable form, so it can be completed and signed electronically.
• Supervisors should meet with the employee to discuss the review. Use the questions to
guide the conversation and discuss the feedback section in detail.
o We recommend for the time being that if possible that managers have
performance review conversations with their employees electronically (via
Microsoft Teams, Zoom, or other virtual meeting programs).
• The completed and digitally signed form should be sent via email to
PerformanceReviews@cityofdenton.com. You DO NOT need to complete a status sheet.
We hope that this update will help make the review process easier for everyone involved. Look
