DEADLINE FOR THE HIP REQUIREMENTS
The Annual Health Assessment (AHA) blood draw and follow-up visit must be completed no later
than Friday, July 30.
The 2020/2021 HIP requirements include two components to qualify for the 2022 premium
differential:
1. Employees MUST complete the annual health assessment (AHA), which includes a
fasting blood draw and questionnaire, at the City of Denton Employee Health Center
(“Clinic”); and
2. Complete a follow-up appointment with a Clinic provider to review AHA results. The follow-
up appointment may be scheduled as a tele-visit or onsite appointment.
Employees do not have to wait 11 months from last AHA blood draw date for the 2020/2021 HIP
period and the AHA date you choose will “reset” the date for future HIP incentive years. New
hires, with health coverage effective July 1 thru October 1, will have the opportunity to complete
the HIP requirements between Monday, Aug. 2 and Friday, Oct. 29 to qualify for the 2022
premium differential.

Make your appointment NOW! AHA appointments have been opened through the July 30th!
Appointments fill up fast!
Remember, you have to have both visits (blood draw and follow-up) completed by Friday, July
30th.
HOW DO I SCHEDULE AN APPOINTMENT?
You have three options to schedule your appointment:
1. Log into the CareHere website (www.carehere.com)
2. Call the CareHere Help Center at (877) 423-1330
3. Use the CareHere app
I DON’T HAVE ACCESS TO THE CAREHERE WEBSITE! WHAT DO I DO?
You must be registered on the CareHere website to access the scheduler. The access code for
the registration process is: CFDT2.
The registration process will set up your username and password and create your patient
electronic medical record (EMR).
