Definition: Under general supervision, responsible for providing comprehensive case management services, resource management, and other support to individuals identified by the Homeless
Outreach Team.
Essential Functions:
Maintains and monitors an assigned caseload of participants, through casework/resource management and referral, and skill teaching/refinement to help them obtain and maintain housing.
Provides support to participants in a residential setting or to those seeking living arrangements with supplemental service support.
Partners with Homeless Outreach Team officers to contact individuals and complete the Coordinated Entry intake assessment.
Assesses initial program participant needs, based on individual strengths and available resources.
Participates in the setting and maintaining goals with program participants, for inclusion in the individualized service plan.
Conducts weekly meetings with participants on a consistent basis, including house visits.
Collaborates with agencies and nonprofit service providers to facilitate case coordination and information sharing throughout the City and avoid duplication of services.
Tracks participant enrollment, services, and referrals in the Homeless Management Information System (HMIS) database, ClienTrack.
Assesses and identifies gaps in service delivery and recommends alternative action plans.
Provides documentation in a timely manner, which may include progress notes, outcome evaluation, service plans, incident reports and evaluations.
Provides crisis intervention when necessary.
Communicates effectively with team and supervisor about concerns regarding participants.
Participates in outside professional groups and committees.
Attends weekly team meetings and provides required reports.
Maintains regular and punctual on-site attendance.

Minimum Qualifications / Acceptable Equivalency:
Master’s degree in social sciences or related field (social work, sociology, or psychology)
Must be a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
Two years of case management experience
OR
Any combination of related education, experience, certifications, and licenses that will result in a candidate successfully performing the essential functions of the job

Knowledge of basic principles and methods of rehabilitation
Previous experience in a not-for-profit setting
Conditions of Employment:
Must have and maintain a valid Class “C” Driver’s License and valid state required minimum automobile liability insurance prior to employment (must obtain Texas Class “C” driver’s license
and state required minimum automobile liability insurance within 90 days of hire per state law)
Must successfully complete the employment process which will include the following: fingerprints, a drug screen, criminal history background check, and social security number verification
Must meet the Denton Police Department’s Civilian Hiring Standards
