lock-Operator made Gate repair during inspection. On 6/19/18,RW conducted a follow-up inspection of the well site.All items requiring
—further attention from the Operator were not yet resolved.The Operator was contacted and/or another follow-up inspection will be conducted. On
—7/19/18 RW conducted the 2nd Follow-up Inspection with Operator representative present during inspection. Issues with the well site remain-
-unresolved.Operator is evaluating equipment conditions and will summit a plan with a time frame to resolve issues. Additional inspections will be
conducted via spot inspections
