Inspection Comments
Signs need correct RRC ID. #1 WH ss fitting loose and venting 10% lel. Tank Battery signs need RRC ID#. Tank out of service not
disconnected from In-service tanks and found venting thru hole. DV Controllers found venting at 10% lel. Containment Liner needs
repair. On 1/7/19 RW conducted a re-inspection,WH Leaks were repaired DV controllers were repaired. Hole in tank repaired, Signs
—and Containment liner issues not addressed. On 3/4/19, RW conducted a re-inspection. Upon arriving on location a leak from the
_containment liner was discovered. The liquid was checked with a salinity analyizer and found to be greater than allowable. Operator
advised that the spill would be reported to the RRC and remediation would begin. Issues with the signs were resolved
