needed. Tank 1 has holes in top of tank needs repair. .
On,11/8/19,RW inspected well.All inspection items were documented on the Inspection Checklist and Operator was notified of issues requiring action.No
immediate actions were needed. On 12/10/19,RW conducted a follow-up inspection of the well site.All items requiring further attention from the Operator were not
yet resolved.The Operator was contacted and/or another follow-up inspection will be conducted. On 1/29/19,RW conducted a 2nd follow-up inspection of the well
site. All items requiring further attention from the Operator were not yet resolved.
