The following are clarifications to the scope of services/specifications:
Disability:
1. Please confirm if the City prepares W-2s for STD claimants, or if the City requires the carrier
to do so.
The City requires the carrier to prepare W-2s for STD claimants.
2. Please confirm whether the employee’s STD premium contributions are paid with pre or post
tax dollars.
Premiums are paid with post-tax dollars.
a. If paid with pretax dollars, does the City require that the carrier pay the FICA match or
does the City intend to pay the FICA match?
N/A
3. Please confirm if the City has telephonic claims service on the STD currently.
Yes, currently the City uses a telephonic claims service.
4. Over 146 employees have STD amounts that do not match the salaries listed; please clarify.
Not sure why, but the STD benefit amounts should be 60% of weekly salary.
5. What were the inforce rates with The Hartford?
This will be provided in the next addendum.
6. Did any plan changes occur when transitioning from The Hartford to Dearborn National?
Guarantee Issue increased on Basic Life class 2. Spouse GI increased from 30,000 to 50,000.
7. Is there a LTD Reserve for the Dearborn open LTD claim #15959?
It’s listed as “Approved” and the open reserve amount is blank. Dearborn’s clarification will
be provided in the next addendum.
8. When did the last open enrollment occur for the Life and Voluntary STD?
We did an open enrollment for the 1/1/16 for Supplemental Life only, not Voluntary STD.
9. Is an updated Dearborn National experience report available?
It is 6 months old. Additional information to be provided by Dearborn will be in the next
addendum, if possible.
10. What are Dearborn National’s renewal rates and renewal rate guarantee?
The renewal has not been received and will be provided through the RFP process.
11. Is the City of Denton happy with Dearborn National’s service?
The City will not respond to this question.
