Please note that the actual contract third renewal period is effective November 15, 2019 through
November 14, 2020. Please keep in mind, according to Section 5 of the contract’s Exhibit A, Special
Terms and Conditions, requests for a price change must be submitted in writing at least 60 days prior to
the contract renewal date, which falls on the annual anniversary of the contract’s effective date.
Additionally, supporting evidence for need of such increase must be included with the request.
