On June 30, a sewage line was obstructed and caused a wastewater backup into the jail and (newly constructed)
evidence area at CHE. This damaged recently constructed and renovated space, required serious remediation,
and build back to meet construction schedule.
On July 5, a shelter client hit a sprinkler head in the emergency bunk room, causing water damage to all 10 bunk
rooms and the hallway of the Loop 288 shelter. To ensure appropriate level of service to shelter guest, a
contractor must remediate all water, test for water intrusion, remove base cove, replace base cove, and perform
spot sampling to ensure safe occupancy of staff and guests.
On July 5 at 7am, Service Center warehouse staff reported water coming through the ceiling into downstairs
warehouse offices. Upon investigation, we found that the plastic water line to the refrigerator/ice maker in the
upstairs breakroom had burst, leaking water into the breakroom, Lori Hewell’s office, and the downstairs
warehouse offices. The water was turned off.

OVERVIEW
BMS CAT opened a file on Denton Homeless Shelter (Client) in Denton, Texas on July 5, 2023 for
damages related to a water release that occurred on 07/05/2023. Client representatives, David
Moore and Project Director, Jesse Zimmerman met on 07/05/2023 and inspected the impacted
areas of the building.
DISCUSSION
BMS CAT Representative Jesse Zimmerman met onsite to walk the affected areas and observe
the damages. Visible damages identified in this assessment included but not limited to the
following building materials: Ceiling tiles, ductwork insulation, drywall ceilings, wall board, batt
insulation, cove base, FRP and flooring materials.
Preliminary Affected Areas Identified on Moisture Assessment:
• A1 – A10 Sleep Rooms and Main Hallway
SCOPE OF WORK
The project will be conducted to maintain normal or near normal operations in the facility
(8:00AM to 6:00PM). The extent of damage was surveyed and this scope of work has been
developed based on similar work performed at other facilities and water damage remediation
work practices. The scope of work provides a roadmap for the systematic drying of damaged
materials of the building in a manner that does not jeopardize the safety of occupants and
visitors. The focus centers on drying of building elements; while minimizing business
interruption.
As needed, Client will develop Interim Life Safety Measures (ILSM) for the areas of the building
pertaining to the project.
Pending any unforeseen issues, we anticipate the water remediation phase of the project to
take 9 days to complete. BMS will keep Client posted, if said issue is to arise with ample notice
prior to anticipate project completion.
REMEDIATION PROTOCOL & PROCEDURES

Drying Strategy:
• To accommodate the power demands of the drying equipment, BMS will setup a
temporary generator on the North side of the building. BMS will provide refueling, as
needed.
• BMS will deploy air movers and dehumidifiers throughout the impacted areas.
• BMS will track the drying process and provide daily reporting of the progress.
Particle Control & Building Prep:

Wallboard – Controlled Demolition:
• BMS will remove the vinyl cove base from the impacted walls and core holes for wall
cavity drying.

replacing the vinyl cove base that we removed.
SCHEDULE & MANPOWER
BMS CAT will provide up to (1) Project Director (1) Project Manager, (2) General Restoration
Supervisors and (1-2) Restoration Technicians per day, as needed, to perform the work
described above.
BMS CAT will mobilize on July 05, 2023, and work will commence on July 5, 2023. BMS CAT
will work one (8-10) hour shift each day, with an estimated crew of (1-2) in order to complete
the work in a timely manner. Additional personnel may be added, as needed, to complete the
work. BMS CAT estimates that this project will be completed by end of day, 07/13/23.
HEALTH & SAFETY
BMS Remediation personnel performing work on the project will have participated in our
Safety Specific Orientation on Particle Control Policies and Procedures. This training also
includes information on interim life safety measures and site-specific safety issues.
BMS will conduct daily safety sign in sheet for all staff, sub-contractors and vendors onsite.
These sign in sheets will be in our Binder that stays in our onsite field office and is available for
review at all times. Please see BMS Project Manager for location and accessibility. A copy of
this binder will be left with the Client for local, state and federal inspections, post project.

PRICING & INVOICING
These water damage remediation services and building repairs will be performed on a time and
materials basis, per the BMS 2023 Preferred T&M Rate Schedule. As outlined in this scope, the
“Budgetary” price for the work and services will be:
TOTAL BUDGET: $105,000
All BMS CAT costs related to this Scope of Work, as described and detailed in this proposal, will
be tracked and billed on a time and materials basis in accordance with agreed to Time and
Material Rate Schedule. Client will receive a copy of all supporting documentation for review
and verification purposes. Client will only be invoiced for services provided and actual costs
incurred in accordance with the rate schedule.
All applicable local, state and federal taxes, and required permits, will be applied and invoiced
in addition to the amounts stated above as required by law.
An initial 1st Draw Invoice of $50k will be submitted and is due upon receipt. Progress invoices
will be provided throughout the course of the work, as applicable. Payment terms for all

invoices will be NET-10 days from the date of the invoice.
Throughout the project, accumulated costs will be monitored. In the event that the scope

OVERVIEW
BMS CAT opened a file on Denton Jail (Client) in Denton, Texas on June 30, 2023 for damages
related to a water release that occurred on 06/30/2023. Cumulatively, the water from the sewer
back-up impacted about 8,000 sq ft of the building. Client representatives, David Moore and BMS
Project Director, Jesse Zimmerman met on 06/30/2023 and inspected the impacted areas of the
building. Following that meeting BMS brought in Colton Turner and Jacob Colson with Ensolum
to oversee the environmental impacts.
DISCUSSION
BMS CAT Representative Jesse Zimmerman met onsite to walk the affected areas and observe
the damage. Visible damage identified in this assessment included but not limited to the
following building materials: vinyl cove base, FRP, wall board, batt insulation, millwork, lockers,
shelving and flooring materials.
Preliminary Affected Areas Identified on Ensolum’s Moisture Assessment:
“AREA A” (EAST), “AREA A” (WEST), “AREA B”, “AREA D” (NORTHEAST), “AREA D”

SCOPE OF WORK
BMS will oversee all Particle Control Containments and maintain Client Protocols/Procedures for
the duration of the project. This includes remediation and cleaning of impacted materials. As
needed, all work within our erected containment will be performed in accordance with CDC, APIC
and EPA criteria.
The project will be conducted to maintain normal or near normal operations in the facility
(8:00AM to 6:00PM). The extent of damages was surveyed and this scope of work has been
developed based on similar work performed at other facilities and water damage remediation
work practices. Further technical specifications will be provided by Ensolum. The scope of work
provides a roadmap for the systematic removal of damaged materials from the building in a
manner that does not jeopardize the safety of occupants and visitors. The focus centers on
remediation of building elements; while minimizing business interruption.
As needed, Client will develop Interim Life Safety Measures (ILSM) for each containment
pertaining to the project.
Proper notifications will be displayed on all containments, as well as any applicable interim life
safety communication. As needed, negative pressure enclosures will be used in conjunction
with HEPA filtered air filtration units, which will filter up to 99.99 of the airborne particles down
to 0.3 microns. As needed, a visual inspection will be conducted for each work area under

containment three times a day. As needed, we will document the conditions using our APIC
based containment inspection checklists to verify the integrity of each negative pressure
enclosure and ensure contractor personnel are following particle control policies and
procedures. All remediation work will be performed in accordance with CDC, APIC and EPA
criteria, as previously stated.
Ensolum will be responsible for all Post Remediation Verification and Environmental Clearances
of all affected areas prior to being released back to Client. Pending any unforeseen issues, we
anticipate the project duration to take 16 days to complete. BMS will keep Client posted, if said
issue is to arise with ample notice prior to anticipate project completion.
REMEDIATION PROTOCOL & PROCEDURES
BPP Relocate – Pack & Move Contents:

Millwork, Plumbing, Electric:
• Lower millwork will be detached, so flooring demo can be performed. Millwork with
sinks will have the plumbing disconnected prior to detachment.
• BMS is handling the detachment of the millwork, countertops and built-in appliances.
Particle Control & Building Prep:
• BMS CAT will conduct all activities according to the requirements of Ensolum.
• Provide additional HEPA-filtered air-filtration units as needed.
• Construct dust barriers as directed.
• Maintain dust control during remediation using HEPA-Vacuums, tacky mats and wet
methods.
• Building prep includes: detachment or sealing of the wall hangings or signage (as
directed), sealing off unaffected areas, sealing the HVAC registers and returns, protect
flooring (as needed), cover/protect exterior windows, etc.
Wallboard – Controlled Demolition:
Wall board cuts will be conducted where noted on Ensolum’s protocol. Controlled demolition
procedures will be used:
• A dust barriers will be constructed in areas where wallboard will be removed.
• Wallboard removal will be conducted by unscrewing or carefully prying from studs. HEPA-
filtered dust collection devices will be used with electric saws - Debris generated will be
placed directly into 6-mil poly bags.
• Insulation will be removed if present - Insulation will be placed directly into 6-mil poly
bags.

• All 6-mil poly bags will have the air evacuated via a HEPA vacuum - The bag will be “goose-
necked” and sealed with duct tape - All 6-mil poly bags containing waste materials will be
double bagged upon exiting the negative-pressure enclosure.
• Moisture-damaged materials will be removed from metal structural members with a
HEPA vacuum – moisture/fungal damage will be removed.
• Structural members will be wiped with an EPA registered microbial disinfectant.
• Debris from floor wall track will be removed with HEPA vacuum.
• Structural members that have lost their integrity should be replaced.
• Materials in 6-mil poly bags will be removed in carts with tight cover - Exterior of carts or
bags will be wiped with an approved microbial detergent before removing from site.
• Additional inspection of wall cavity will be conducted for evidence of unanticipated
moisture damages.
• Detail cleaning will be conducted upon completion of demolition.
Flooring:
• Impacted carpet flooring will be removed per the moisture maps provided by Ensolum.
General Cleaning Procedures:
• Cleaning will be conducted from the top down.
• All surfaces will be HEPA Vacuumed.
• All surfaces will be wiped with an EPA registered disinfectant.
• All project chemicals SDS will be onsite through the duration of the project.
SCHEDULE & MANPOWER

the Scope of Work, BMS CAT will obtain a Statement of Work Complete signature indicating that
the project has been completed to the satisfaction of Client.
PRICING & INVOICING
These water damage remediation services will be performed on a time and materials basis, per
the BMS 2023 Preferred T&M Rate Schedule. As outlined in this scope, the “Budgetary” price
for the work and services will be:
TOTAL BUDGET: $280,000
All BMS CAT costs related to this Scope of Work, as described and detailed in this proposal, will
be tracked and billed on a time and materials basis in accordance with agreed to Time and
Material Rate Schedule. Client will receive a copy of all supporting documentation for review
and verification purposes. Client will only be invoiced for services provided and actual costs
incurred in accordance with the rate schedule.
All applicable local, state and federal taxes, and required permits, will be applied and invoiced
in addition to the amounts stated above as required by law.

An initial 1st Draw Invoice of $140k will be submitted and is due upon receipt. Progress
invoices will be provided throughout the course of the work, as applicable. Payment terms for
all invoices will be NET-10 days from the date of the invoice.
Throughout the project, accumulated costs will be monitored. In the event that the scope
changes or unforeseen costs are incurred, BMS CAT will notify the Client prior to proceeding.
