The respondent’s submission shall have accurately described your understanding of the objectives and scope of the requested products and services and provided an outline of your process to implement the requirements of the Scope of Work and Services. It is anticipated that the scope proposal submission will include, at a minimum, the following: Units must be installed within 24 hours of request. The awarded contractor will be required to meet the minimum insurance requirements as attached, including general liability, auto liability, and workers’ compensation. Service shall include the complete removal of wastes, replacement of appropriate chemicals to assure masking of odors and cleaning and disinfection of units, replacement of supplies, repairs to units and equipment, and all other services necessary to maintain a neat and sanitary unit. All units will be supplied and restocked after each servicing with a minimum of 1500’ of two- ply toilet paper and a scented odor disk as needed The successful contractor shall be responsible for repairs and replacement of their units due to damage resulting from vandalism, accident, storm, or otherwise. Repairs or replacement of the unit shall be made by the contractor within twenty-four (24) hours of notification to ensure uninterrupted service. Pricing shall include delivery, set-up, supplies, removal, urinals, servicing as specified and all fees, including loss damage waivers. The City will not be held responsible for damage to units and has opted to purchase the loss damage waiver. The successful contractor must be available to respond to weekend and holiday service and/or problems. The successful bidder will provide the City of Denton with one or more emergency “ON CALL” telephone numbers. Emergency response time is within four hours of notification by city personnel. Unit locations vary within the city of Denton. Section A-E: Standard, Handicap, Trailer-Mounted Rentals, Hand Washing Station and Additional Service Fees All units shall be serviced weekly or twice a week depending on request Billing shall be monthly. The City will not accept 28 day billing cycles. Units placed less than a month will be prorated based upon the monthly charge. Proposers shall list the fee to move units in the pricing proposal. City staff may tow trailer-mounted units as needed. Contractor will be notified of the new location as soon as reasonably possible in order to prevent interruption of service. Trailer-mounted units must be properly secured to the trailer in order to prevent tipping during use or transit. Units designated for roadway towing must include, but are not limited to, license plates, tail lights and tow chains. For towing on public roadways, Contractor must place the units consistent with any applicable federal or state laws, including but not limited to: • Chapter 393 of the Federal Carrier Safety Regulations. • Texas Transportation Code o Section 545.10 (Safety Chains) o Chapter 502 (Required Registration) o Chapter 521 & 522 (Required Drivers Licenses) o Chapter 545 (Operation of Motor Vehicles) o Chapter 601 (Required Insurance) o Chapter 621 vehicle size (oversize loads) o Chapter 725 (Transportation of Loose Materials) Section F Special Event and Weekend Rentals All units shall be serviced twice daily or more depending on request Proposals shall include pricing for weekly and daily rentals as listed on the line items tab within Ionwave. Section H Miscellaneous Rentals for Special Events Items include Delivery, unloading, set up, pick up, and haul off.
