These have been communicated to ensure that our proposal is based upon the correct information and may be appropriately evaluated. SCOPE OF WORK UES and it’s subcontractor Pacific West will provide 40-Hour HAZWOPER-trained personnel and equipment services for the removal, loading, and hauling and sampling of lead and arsenic impacted soils at the Denton Police Department Outdoor Firing Range, located at 3820 Airport Road, in Denton, Texas (Site). Clean-up activities will be in accordance with the Texas Risk Reduction Program (TRRP) to attain residential Tier 1 protective concentration levels (PCLs). Project Team will include UES Project Manager – 1 Pacific West Site Superintendent - 1 Heavy Equipment Operators - 2 Laborers - 2 Environmental Scientists - -2 SWPPP Inspector -1 Pacific West’s on-site personnel(s) will have at least the following training and certifications: • 40- Hour HAZWOPER, Equipment Operations Safety, First Aid, CPR, Lead and Silica Awareness. General Conditions, Pre-Construction, Mobilization & Demobilization • UES and Pacific West will utilize our previously submitted site-specific health and safety plan (SSHSP) prior to the implementation of the proposed work. • Pacific West will provide utility locate service by means of Texas 811 before any excavation activities. • Pacific West will request an extension of the existing Clear & Grading permit from The City of Denton. Pacific West will be responsible for the control and prevention of storm water from entering the excavations, impacted soil stockpile and leaving the project site with best management practices per the plan. UES’s SWPPP Inspector will inspect the area per the SWPPP requirements. • Pacific West will provide and service one (1) sanitation portable restroom for the duration of the project. • Pacific West will install and maintain temporary BMP’s for the duration of the project, per the SWPPP. Estimate at 1,500 feet of silt fencing. • Pacific West will install vegetation stabilization of the site, consisting of hydroseeding with TXDOT Region 2 seed and a nitrifier mix for 2 acres. Concrete Recycling (Demo, Removal, Loading, Transportation & Disposal) • Pacific West will load and haul one (1) load of concrete debris rubble to a recycler. It is our understanding the concrete debris will be considered inert.. The minimum trucking charge has been applied to this line item.. • We will provide the City of Denton a copy of the load ticket to document proper disposal of waste removed from the site and to serve as project closeout documentation. Soil Excavation & Staging for Characterization • Utility Company is responsible for removal of the existing utility pole and placement of the new replacement pole. Utility company anticipates it will take one day for set up of new pole and removal of existing pole. Pacific West will excavate and stockpile up to 1,500 cubic yards of soil remaining onsite under the existing utility line. These estimated soil quantities are provided for reference purposes only and do not constitute a guarantee of final quantities, which may be greater or less than the estimated amount. • Due to safety concerns with the location of the powerlines and pole that run along the berm, Pacific West will utilize a bulldozer to push the berm towards the stockpile area. • Pacific West will protect and preserve the integrity of the groundwater wells located on the Site. • Pacific West will stockpile the impacted soil in a 50-feet X 50-feet at 5’ – 5’-2” thick grid pattern or 500 cubic yard stockpiles then one soil sample per grid will be collected and analyzed for lead (Performed by UES). Confirmation sampling will only occur after all work in a given area is complete. UES previously sampled the soils for this area and it is a Class II. • Pacific West understands the stockpile method is to first lay plastic sheeting down where the stockpile is to be located. Excavated soil is then placed in a ring on the plastic, leaving several feet of free plastic around the edges such that the plastic can be folded over the soil ring to create a plastic-encased berm. The rest of the excavated soil can then be placed inside of the soil berm. Another plastic sheet is used to cover the stockpile such that dust does not escape, and water does not enter the stockpile. Class II Non-Hazardous Waste Disposal (Loading and Offsite Disposal) • UES personnel will be responsible for all sampling, analysis, profiling and manifesting of waste. UES will utilize the previous soil samples results collected from this area • Our pricing assumes this Class II Non-Hazardous Waste will be acceptable for disposal as class II solid waste at Republic Services (RS) Camelot Landfill in Lewisville, Texas. Manifests will be provided by UES. • The Class II Non-Haz soil will be loaded into 18-wheel end dump trucks, transported and disposed of at RS Camelot Landfill. • At the completion of the waste disposal, Pacific West will provide copies of all trucking tickets to serve as project closeout documentation. Republic Waste will supply the manifest directly to the City of Denton. For cost estimating purposes, it is assumed that 5 locations (for ten samples) will be grided and sampled at 0-1 foot, and 1-3 feet. The sampling will occur over a two day period. Soil samples will be analyzed by EPA Method 6010D (lead only) Rush TAT • If needed up to four (4) samples will also be analyzed by EPA Method 6010D (Total RCRA Metals) Rush TAT . • The soil samples will be sent to an approved laboratory for rush turnaround time. The laboratory analytical results and a location map will be provided to the City of Denton. Our proposal is conditioned upon the following: • All work will be performed Monday-Friday, between 6:00 a.m. and 6:00 p.m., excluding holidays. • All work will be performed in one mobilization/demobilization. • Bid Bond – Bid security in the form of a bid bond is NOT included in Pacific West’s proposal. • Performance and Payment Bond – Performance and Payment bond is NOT included in Pacific West’s pricing and can be added at an additional fee. • Portable sanitation restrooms are included in pricing. • SWPPP plans, design, permitting, or erosion control measures are not included in proposal, as the existing one will be utilized.. • Pre/Post Surveying or Construction Staking is not included in proposal. • The utility company will provide relocation of any existing utilities or structures and disposal of the existing utility pole. • Hard excavation or rock excavation is not included in the proposal. • Traffic Control (plan design, temporary fence/barricades, variable message boards, flagman, ADA compliance, temp traffic signals) is not included in the proposal. • Night Work (5:00pm - 5:00am) or weekend work is not included in proposal. • Dewatering equipment for accumulation of water in open excavation is not included in the costs. • Geotechnical and compaction testing is not included in scope of work if needed.. • UES and Pacific West will not incur delays beyond our control. • Decontamination pad of Pacific West equipment or 18-wheel dump trucks entering/exiting construction zone is not included in the proposal. • Import fill, backfilling, site restoration (including watering, maintenance, and regrowth of grass) is not included in this proposal. In the event that any assumptions prove to be deficient and / or conditions have been changed UES would appreciate the opportunity to modify our proposal accordingly. This proposal is valid for a minimum of thirty (30) days from the date of this Proposal and UES reserves the right to withdraw this. Any work performed outside of the proposed scope of work and/or schedule or any work necessitated by material changes in project conditions will be conducted and invoiced in accordance with UES’s and Pacific West’s standard time and materials rate schedule in effect at the time of performance unless otherwise agreed to in writing by both parties.
