Mayhill Road Denton (Denton County), Texas Dear Ms. Reeder: In accordance with your recent request, Solutient GeoSciences, Inc. (Solutient) is pleased to submit the following proposal for providing professional services to plug two piezometers located at the above referenced landfill facility. Scope of Services As we understand it, piezometers EB11-W1 and EB11-W2, which are constructed of 2-inch diameter PVC casing and screen, have recently been damaged, including removal of the surface completions. Due to the presence of heavy equipment operating in the area, the City wishes to plug the piezometers. As such, the piezometers will be plugged in accordance with 16 Texas Administrative Code (TAC) §76.104 by a licensed water well driller under the direct supervision of a Solutient Hydrogeologist. Plugging activities will include an attempt to remove all casing and screen followed by plugging with bentonite pellets/chips and hydration. Upon completion of the field work, State of Texas plugging reports will be completed and submitted to the Texas Department of Licensing and Regulation (TDLR), as required. Also, a letter report summarizing the plugging activities will be prepared and submitted to the Texas Commission on Environmental Quality (TCEQ) municipal solid waste permits section. As part of this work project, Solutient will provide a Hydrogeologist experienced with piezometer plugging activities to directly supervise the field work. The plugging activities will be performed by C&S Lease Service LC as an independent subcontractor to Solutient. C&S Lease Service LC is a state of Texas licensed water well driller and has performed similar work at the landfill facility previously. Solutient will also conduct daily safety meetings to insure the work is performed in a safe manner. Permission to enter and access the site with single-drive and tandem-drive axle trucks and track-mounted equipment will be provided by the City. It is estimated that the total amount of this proposal should not exceed $10,022.50. You will be notified if unforeseen conditions are encountered or there is a necessity to change the scope of work. In the event additional services are required beyond those detailed in this proposal, such will be performed on a time and materials basis. Additional work will not be performed without first obtaining the City’s approval of any additional costs. Invoicing will be performed on a monthly basis and will be based on the actual work performed and the unit prices shown in the attached Probable Cost Estimate. Commencement and Completion It is estimated the work can be performed within one to two weeks upon receiving the City’s authorization to proceed, and we estimate the field work can be completed in one working day. If this proposal is agreeable, please sign and date it, and forward a copy to my attention. Issuance of a purchase order will also serve as authorization to proceed.
