The following are changes to the specification sheet:
Q1: In the Statement of Work, “Tracking”, reference is made to "Every unit
must have a unique identifier". Is this tracking by shipping container, or by
glove, pair of gloves or sleeves?
A1: Unique identifier is by pair of gloves or sleeves.
Q2: What brand is approved for sleeves?
A2: Approved brand is Salisbury, contoured.
Q3: What are the expected additional services listed on the Price Sheet?
A3: Any additional services Vendor provides or required in the duration of the
contract.
Q4: On the pricing sheet columns C 7-10 and E 27-37 reference a unit of
measure as each. For testing gloves is "each" per glove or per pair?
A4: Unit of measure “each” refers to per glove or per sleeve.
Q5: On the pricing sheet “City of Denton requested completion schedule”,
column E 7-10; what is the requested completion schedule for items
listed?
A5: City of Denton request Monday of last week of each quarter for
Completion of Testing. Please state After Receipt of Order (ARO) on
Pricing Sheet. City’s expected ARO is 30 days. Revised Pricing Sheet
attached herein.
Q6: For the pricing of replacement gloves should we include proof testing in
the cost of the new glove?
A6: Pricing shall include all costs to deliver goods as specified FOB
Destination. Revised Pricing Sheet attached herein.

Q7: In reference to Pricing Sheet, would replacement (quoted) Sleeves be
both Class 2 and Class 3?
A7: Replacement sleeves bid includes both Class 2 and Class 3.
Q8: Would replacement (quoted) sleeves be Molded or Dipped?
A8: Replacement sleeves mostly require Dipped. Special circumstances may
require molded.
Q9: Would replacement (quoted) sleeves be all one size or various sizes (ie
Regular, Large, x-Large)?
A9: Various sizes including a “True Short” (-22”).
Q10: Please explain what price to enter for "Cost of Additional Service Years"?
A10: Any additional services Vendor provides or required in the duration of the
contract. Enter price for Year 1, Year 2, and Year 3 of contract duration.
Q11: The Statement of Work indicated that the Vendor must pick up and deliver
all goods at the Denton Texas location. Would the Vendor need to pick
these up or can Vendor organize shipping?
A11: State of Work, Transport, states “Vendor must pick up and deliver all
goods at DME’s location at Spencer Road, Denton, TX”. To clarify, the
Denton location is 1659 Spencer Road, Denton, TX, 76205. “Curbside”
service is required for pick up and delivery; no shipping.
NO OTHER CHANGES AT THIS TIME.
