Project Description: Lake Lewisville WTP Improvements Phase II
Description of Services Added/Deleted:
1. Design Separate Buildings for HSPS Electrical Switchgear and Administration Facilities. 2. Extend active site boundary east to accommodate new Administration building. 3. Provide related surveying, geotechnical, and structural design services.
Deliverables:
Compensation shall be adjusted as follows:
Additional estimated effort detailed on the attached table.
Original Contract $1,529,180
Amended Amount $21,965
Revised Total Contract $1,551,145
Schedule shall be adjusted as follows:
Design phase will be extended approximately six weeks, to a total of 11-1/2 months.
The above described services shall proceed upon return of this Contract Change Authorization. Services
will be billed as they are done. All other provisions, terms, and conditions of the agreement for services which are not expressly amended shall remain in full force and effect.
A contract modification will be submitted.
This Contract Change Authorization will serve as contract modification.
