FNI Project No.: DTN18102 Client Contract Ref.: PO 184278 Date: July 26, 2018 Project Description: Extend Upstream Box to Intersection of PEC-4 and Bernard St. for PEC-4 Phase 3 and 4 Drainage Improvements Description of Services Added: Design an extension of RCB trunk line west 205 LF to the intersection of Mulberry St and Bernard St then south 360 LF along Bernard St ending at the existing box culverts. Design shall include: 1. Stormwater: a. Modeling efforts to determine: i. Required box size for Phase 4 – Extension ii. Addition of arch pipes within Carroll and evaluation of flooding within the vicinity iii. Update box culvert to carry 100-yr flow without utilizing Relic Channel b. 565 LF extension to RCB to the intersection of PEC-4 and Bernard St. c. Curb inlets and 80 LF of laterals to capture runoff within Bernard St. d. Special junction box structure at connection point with PEC-4. e. Update XP Storm model and drainage memorandum with extension. 2. Utilities: a. 440 LF of 8-inch waterline relocation within Mulberry St and 2 - 8-inch waterline crossings within Bernard St. b. 230 LF of 8-inch sanitary sewer line within Bernard St. 3. Transportation: a. 250 LF of pavement repair along Bernard St. including the intersection of Bernard St. and Mulberry St b. Intersection will be designed to be ADA compliant for pedestrians. 4. Survey: a. Additional topographic survey along Bernard St. b. Level B SUE Along Bernard St. c. Two (2) Level A SUE within Bernard. 5. Geotechnical: a. One (1) additional boring near the interstation of PEC-4 and Bernard St. Deletions: Remove Task 6 Construction Phase Services from original contract. Assumptions: Additional design services include the following assumptions: a. No construction phase services are assumed in the amendment for the outlined work above. b. Existing PEC-4 relic channel to remain with any additional work to channel beyond transitions to be considered additional services. c. Pavement to end 20 feet south of the special junction box within Bernard St. d. Pavement design for Mulberry St. to be applied to improvements in Bernard St. New pavement design in Bernard St. to be considered additional services. Deliverables: Box extension and utility relocations shall be correlated into all subsequent deliverables.
