RFP # 6294 ADDENDUM #1 Addendum #1 to be returned with RFP Proposal The following are changes to the bid specifications: NO OTHER CHANGES AT THIS TIME. Please acknowledge addendum on Attachment F of the main RFP document when submitting a proposal. RFP # 6924 Scope of Work – ADDENDUM #1 Supply of Service EXHIBIT 3 RFP # 6294 – Exhibit 3 The scope of work shall be finalized upon the selection of the Firm. The respondent’s submission shall have accurately described your understanding of the objectives and scope of the requested products and services and provided an outline of your process to implement the requirements of the Scope of Work and Services. It is anticipated that the scope of work will include, at a minimum, the following: A. GPS/GIS mapping of Oakwood and IOOF cemeteries to develop a current CAD map of all grave locations, monuments, crypts, trees, roads, fences, and any other permanent landmarks that provide location awareness to accuracy of +/- 4”. Maps to be provided to City on paper (at least one large scale such as 24X36 and two smaller scale such as 11X17) and in digital format for each cemetery. B. Collection of all legible data listed on monuments in a digital database. To include photo, full name of decedent, date of birth, date of death, inscriptions, and veteran status if applicable. Two books of collected, organized data to be provided to City on paper and in digital format for each cemetery. C. Assessment of all monuments to evaluate condition and provide priority based repair schedule. To include monument and base material. Two books of collected, organized data to be provided to City on paper and in digital format for each cemetery. D. Installation of 4 1/2” stainless steel block markers to replace PVC block markers in IOOF division Z 1-31 ½ , division X 42A-47F, division E 7 ½-35 ½, division F 40-42, division G 36 ½-42 division K 36 ½-42 and Oakwood Blocks 7/1, 7/2, 5/1,5/2, 5/3, 5/4. Also in Oakwood Old Part where blocks and quadrants of blocks intersect. E. Annual repair, restoration, and preservation work to damaged monuments. To include leveling, cleaning, and repairs to damaged and broken monuments using historically appropriate preservation materials and techniques as described by Department of Interior standards. F. Proposal for consultation and design of weather proof and vandal resistant visitor information kiosks at Oakwood and IOOF cemeteries. G. Proposal for restoration work to mausoleums at IOOF cemetery and lawn crypts at Oakwood Cemetery using various construction techniques to return them to safe and historically accurate condition using historically appropriate preservation materials and techniques described by Department of Interior standards.
