a. There’s a note under #14 of Attachment A, that states “The
details shall be completed on this form, and shall not point to
another document in the respondent’s proposals”.
Does this refer only to #14 (how the firms meets minimum
qualifications in Section #3), or does it apply to the entire form.

2. 6533-0 Main Document: Solicitation Checklist includes Attachment
C: Schedule of Budget Compliance Form.
a. There is no such form included in the RFQ Package.
Attachment C is the References form (D-F in the checklist are
C-E on the forms).
b. Do we need to provide the Schedule and Budget Compliance
Form? If so, can the City Provide the form?

To answer both questions a & b, the original Attachment C, Budget
Compliance Form was omitted from this RFQ. However, the change
was not made to the RFQ#6533 Solicitation Check List. Even more
confusing, the subsequent Attachments of D through F, were re-
titled to reflect the reduction in documents: Attachment C
References, Attachment D Conflict of Interest Questionnaire and
Attachment E Acknowledgement

3. 6533-0 Main Document: Item #9, of RFQ Statement of
Qualifications:
Statement of Qualification should be no longer than 25 pages. All pages with the
exception of the front cover, the back cover and qualified divider sheets will be
counted in the 25 pages. *** In order to be considered a qualified divider sheet, the
page may have a description (i.e. Project Team, Experience, etc) or a numeral on the
divider tab only. Resumes and/or brochures will be included in the page count. Cover
letters (if bound into the SOQ) will be included in the page count. Please include at a
minimum the following:
A maximum three (3) page narrative as to the firm’s interest, particular abilities
and qualifications related to this project.
The location of your office where the majority of the work on the project will be
performed and the approximate number of employees in that office.
Resumes of key personnel to be assigned to this project with an organizational
chart.
A list and general description of each project that the above-mentioned staff has
performed that would indicate your firm’s ability to perform the work being
considered. At least five (5) project examples should be included. The list of
specific projects shall indicate the professional services and associated costs to
accomplish the project, professional services and construction costs of the
project, along with your client’s name and phone number.
QUESTIONS FROM POTENTIAL RESPONDERS:
a) Does the page limit apply to the entire response, or only to the
item called Statement of Qualifications (fourth in the order of
submission on page 10)?
Yes, per the first paragraph, 6540-0 Main Document, item #9;
the maximum number of pages of the response is 25 pages (not
including the covers or divider sheets). ***

b) On page 5, under Section 9, Statement of Qualifications, it
states that our Statement of Qualifications should be no more
than 25 pages. However, Section # 14 Submittal instructions, a.
Submission Format, item #b on page 7, “Submission shall be
no more than 200 pages in length”. Can you please clarify
which page limit to use for this “Statement of Clarifications”
and all required forms?
Thank you for bringing this conflicting information to our
attention. This is an RFQ template for professional services and
in rare cases used for other services. “Section #14, page 7
Submittal Instructions” is a standard that is incorporated to our
RFQ Template. 200 pages for this RFQ is not allowed and would
require an excessive amount of review and evaluation time.
Because of this question, the RFQ template is being reviewed for
a future change of this section on the RFQ template.
For an absolute answer to your question: Your Statement of
Qualifications should not exceed 25 pages, (excluding the covers
or divider pages) ***
c) Bullet Point 3: ….The submission of “the list of specific
projects shall indicate the professional services and associated
costs to accomplish the project, professional services and
construction costs of the project, along with your client’s name
and phone number” .
RFQ’s seeking professional services for water and wastewater
impact fees update require the services of a professional
engineer and are governed by Texas Local Government Code
§395.014. As a result, this RFQ does not comply with the
aforementioned state law and rules because of the requirement
for the submission of fees or costs on past projects in the initial
phase of selection.
Thank you for bringing this to our attention. The RFQ template
has been revised to clarify this language for the future.
For the purposes of this RFQ, Please refer to RFQ#
6533, Addendum #1, Exhibit C, Revised Section 9.
