Section 1: Items to be Submitted ☐ 1.1 Copy of Deed(s) or Owner Authorization Letter ☐ 1.2 A completed Temporary Right of Entry for Environmental Assessment form, found at the end of this checklist. ☐ 1.3 Associated Fee(s) as listed on the Development Review Fee Schedule ☐ 1.4 ESA Field Assessment Report(s) appropriate for each ESA type and feature assessed. Blank ESA Field Assessment Report forms can be found on the ESA webpage. A project narrative following the standards provided in Section 2. Site map(s) following the standards provided in Section 3. ☐ 1.5 A shapefile of the determined location of ESAs, as defined in the DDC and through the ESA Field Assessment procedure. More information on determining the location and boundaries of ESAs can be found in the ESA Primer, located on the ESA webpage. Please note, this file type cannot be uploaded directly to ProjectDox. You may email the file(s) directly to the Environmental Compliance Coordinator. The following protocols are to be used when mapping the boundaries of the ESA and providing the data: Provide an ESRI Geographic Information System layer (shapefile) with metadata showing the GPS unit accuracy. Environmental Services 901 A Texas Street, Denton, TX 76201  (940) 349-7135 Updated 5/11/2021 ESA Field Assessment Checklist 2 Section 2: Project Narrative Provide a project narrative with a detailed description of the assessment, including, but not limited to: ☐ 2.1 Qualifications of the professional who performed the assessment. ☐ 2.2 A list and description of the ESAs. For complex systems you are strongly encouraged to provide a naming system (e.g. S1, S2 for each stream segment). ☐ 2.2a List ESAs shown to potentially be located within the property boundaries, as indicated on the Official City of Denton ESA Map. ☐ 2.2b List ESAs that were assessed, indicating those ESAs that were found to exist and those that were found to not exist. ☐ 2.3 Supporting evidence and reason for the designation (is or is not an ESA) for each ESA feature that was assessed. Section 3: Site Map Provide a site map, or depending on the size of the property a series of maps, using the following standards to clearly show the location of the ESAs on the property: ☐ 3.1 The location and boundaries of each ESA: ☐ 3.1a Shown to potentially exist, as indicated on the Official City of Denton ESA Map. ☐ 3.1a The determined location and boundaries of the ESAs that were assessed. Include and indicate those ESAs that were determined to not exist. ☐ 3.2 North arrow, written and graphic scale, and vicinity map. ☐ 3.3 Provide a legend if any symbols or abbreviations are used on the map. ☐ 3.4 A title block located in the lower right-hand corner of the exhibit sheet containing: Date of preparation; project name; city assigned project number; exhibit preparer name, address, and phone number; owner and/or developer name, address, and phone number; subdivision name, block and lot. DCAD # Parcel No. Property Owner Name Project Name Project No CITY OF DENTON TEMPORARY RIGHT OF ENTRY FOR ENVIRONMENTAL ASSESSMENT STATE OF TEXAS § § KNOW ALL BY THESE PRESENTS COUNTY OF DENTON § That the undersigned, hereinafter referred to as “Grantor,” for valuable consideration, does by these presents grant and convey to the City of Denton, a Texas home-rule municipal corporation, herein after referred to as “Grantee,” a temporary right of entry onto property described as (Legal Description) at (Street Address) City of Denton, Denton County, Texas, hereinafter referred to as the “Property,” for the purpose of conducting environmental assessments, including but not limited to walking the area, GIS points, taking pictures, small sampling of seeds and/or leaves, and geotechnical testing (small soil cores). Upon execution of this agreement, Grantor will grant Grantee access to the Property for the purpose stated herein, until such time as the purpose is completed and approved by the Grantee, at which time the above described temporary right of entry ipso facto terminates without further action of either party. This Right of Entry shall include the right of Grantee and its employees, agents, representatives, or contractors to access the aforementioned property.
