AN ORDINANCE AUTHORIZING THE EXECUTION OF A CHANGE ORDER TO A
CONTRACT BETWEEN THE CITY OF DENTON AND, DENTON COMMUNITY HOSPITAL
PROVIDING FOR AN INCREASE IN THE CONTRACT PRICE, AND PROVIDING AN
EFFECTIVE DATE
WHEREAS, on 12M- 9~5, the Ctty awarded a contract for RFSP~ 1810 - OCCI JPATIOkNAL
HEAl.TH lqlJJLSE in the amount of $23+4DD~0 for 20 hours ger ~veek servmes, and
WHEREAS, the Ctty Manager hawng recommended to the Councd that a change order be
authorized to amend such contract wtth respect to the scope and price and satd change order being
tn comphance wtth the reqmrements of Chapter 252 of the Local Government Code, NOW,
THEREFORE,
THE COUNCIL OF THE CITY OF DENTON HEREBY ORDAINS
SECTION I. That the change order tncreas~ng the amount of the contract between the Ctty
and DENTON COMMIJNITY HOSPITAL copy of whtch ts attached hereto, in the amount of

ts hereby approved and the expendtture of funds therefor ts hereby authorized The contract amount
is amended to read $52,680 00 for 40 hours per week servmes through December 31, 1996

RECOMMENDATION. We recommend that the contract for Occupattonal Health Nurse Servmes
be rewsed to expand the Occupatxonal Health Nurse Servxces from twenty (20) hours per week to
forty (40) hours per week beg~nmng April 17, 1996
SIIMMARY. The ordinance authonmng the ~mplementat~on of an Occupational Health Nurse
(OHN) was approved on December 19, 1995 for 20 hours per week for a total expen&ture of
$23,400 00 for the first year Thxs change order would authorize an xncrease of the hours to forty
(40) per week and the amount to $52,680 00
