AN ORDINANCE AMENDING AN AGREEMENT BETWEEN THE CITY OF DENTON AND
SANUS TEXAS HEALTH PLAN, INC TO EXTEND THE CONTRACT BY ONE MONTH,
TO PROVIDE FOR REVISED FEES FOR THAT MONTH, AND TO PROVIDE FOR
TERMINATION, AND PROVIDING FOR AN EFFECTIVE DATE
THE COUNCIL OF THE CITY OF DENTON HEREBY ORDAINS
SECTION I That the Contract between the City of Denton and
Sanus Texas Health Plan, Inc shall be amended as set forth in the
attached Amendment
SECTION II That this ordinance shall become effective im-
mediately upon its passage and appro~

AMENDMENT OF AGREEMENT BETWEEN
THE CITY OF DENTON AND
SANUS TEXAS HEALTH PLAN, INC
Recitals
On October 17, 1989, the City of Denton (City) and Sanus Texas
Health Plan, Inc (Sanus Texas) entered into an agreement (the
Contract), a copy of which is attached, providing for group medical
coverage for city employees The Contract was effective on
December 1, 1990, and runs for consecutive one year terms unless
terminated by either party giving written notice to the other sixty
days prior to the anniversary date
The City is now soliciting bids from other health care providers
for the purpose of awarding a new contract for group medical cover-
age for City employees, to be effective on January 1, 1991 To
provide for uninterrupted coverage of medical benefits for City em-
ployees, the City and Sanus have agreed to amend the Contract be-
tween the parties as provided herein
AGREEMENT
In accordance with paragraph VIII, B, ("Amendment") of the Con-
tract, the City and Sanus Texas agree as follows
1 Notwithstanding any other provision of the Contract, the
term of the Contract beginning on December 1, 1990, and
ending on November 30, 1991, is extended so as to terminate
at 11 59 p m on December 31, 1991, and shall not continue
thereafter as otherwise set forth in paragraph VIII, A
("Term") of the Contract, each party expressly waiving the
requirement to give sixty days written notice prior to the
termination date, as otherwise provided in paragraph III,
B, ("Termination of Group") of the Contract
2 Notwithstanding any other provision of the Contract, the
monthly premiums to be paid by the City to Sanus Texas for
the month of December of 1991, as extended by this Amend-
ment, shall be increased to the following amounts
