Call to Order A Quorum was established. Sheryl English, Chair called the meeting to order at 11:07a II. Introductions Sheryl English, Chair welcomed committee and staff. Members and staff introduced themselves. III. Oath of Office (if needed) All Oaths are current. IV. Approval of Minutes from November 14, 2016 Minutes from November 14, 2016 were presented for Approval. Christy Wood identified that the date in the header needs to be amended from 2015 to 2016. Christy Wood moved that the minutes be approved as corrected. Pam Barnes seconded the motion. The motion carried. V. Human Services Advisory Committee a. Conflict of Interest Disclosure Pam Barnes disclosed that she is on staff at Health Services of North Texas, a current recipient and a current applicant for the 2017-18 grant. VI. Human Services Applicant Presentations Sheryl English opened discussion The following notes from the agency funding presentations are provided for committee information and review. TAB Applicant Organization Guest(s) Presentation Time 15 Health Services North Texas Doreen Rue,ChiefExecutive Officer 11:30a-11:40a Louise Weston-FenilL Director ofPrograms Pam Barnes was excused from discussion due to her relationship to the agency. The agency was provided with five minutes to present information to support its funding request. Health Services of North Texas discussed the organizations growth by providing an update of the organizations services Year-end. The data supports the need for services seeing 14% more patients in 2016, 22% more services provided and more than 600 new patients. In surveys 95% of patients indicated they would refer someone to Health Services of North Texas. Agency provided information about the prescription assistance program sharing that they were able to help supply more that $2 million in prescription medications at an average cost of$139 per meds. Agency affirmed that healthy patients have an improve quality of life which benefits everyone in Denton. Agency described new services being offered, the tele-medicine program that is assisting with psychiatric care and mental health assessments. Agency covered its strategic and funding goals including construction of a new building and expansion to 9 new exam rooms increasing care. HSAC members took five minutes to ask the agency questions. Agency was asked how many full time doctors were on staff. The agency shared that there are 3 full-time doctors. The agency was asked about the Mental Health information shared in the presentation. Agency provided more detailed information about the tele-medicine program serving providing more than 5000 mental health visits. There was additional discussion about the benefit of tele-medicine for medication management. There was a brief transition to the next presentation. TAB Applicant Organization Guest(s) Presentation Time 11 Denton County NU VR Lana'West,Administrator ofIDD Services 11:45a-11:55a Lora Blakeslee Atkinson,Business Dev.Specialist The agency was provided with five minutes to present information to support its funding request. Denton County MHMR opened by thanking the committee for the previous grant award. With the $22,000 grant funds they have been able to serve on average 18 clients a month and increasing the quality of life for people with developmental disabilities. Agency shared news of a new collaboration in partnership with UNT that will add additional value to the program and staff through a work study program. Program will benefit from the behavioral health research with UNT health sciences program. HSAC members took five minutes to ask the agency questions. Committee asked additional questions about the UNT partnership seeking information about the total number of practicum students. The agency indicated there would be 3 with matching funds. Agency was ask to explain how it would correct for the unspent funds from the 2015 grant. Agency reviewed that it increased wage to $10 hour. When the agency was asked what inclusion activities are offered and if it had a booth at events, the agency shared that it includes Arts &Jazz Fest, Movies, Music and Sporting events. There was a brief transition to the next presentation. TAB Applicant Organization Guest(s) Presentation Time Ramona Davis,Executive Director 22 Woman to Woman Ev Haman,Executive Assistant 12:OOp-12:1Op The agency was provided with five minutes to present information to support its funding request. Woman to Woman read with its mission statement. Agency discussed that it was is able to deliver programs through community partnerships, volunteers, donations and proceeds from the resale shop. Community partnerships are with organizations that include Arms of Hope, New Day Services, the RSVP VISTA program. Agency reviewed its volunteer program. Agency then shared a case study. The agency indicated it has a goal to expand the pregnancy center, enlarge educational programs and men's outreach programs. It feels it is in alignment with Denton's community goal for strong families. HSAC members took five minutes to ask the agency questions. Agency was asked to clarify what funding would support. It explained that if awarded, funds would support salaries, STD testing and educational program. Agency was asked about how it identifies clients. Agency shared that clients are referred. On average clients are 19-24 years old and come from educational campaigns in the community like TWU campus campaign. Agency was asked if the testing was for more than the two STD's listed in the application like HIV. Agency indicated that the additional tests are too costly. When ask as a new applicant why it was seeking funding from the city, the agency explained that the VISTA researched grants and they decide to apply for funding. Agency was ask about religious affiliations and if its faith based affiliation influenced if clients are provided information about all options available to them. Agency described that two packets are offered. Follow-up question related to HIV testing asked if the agency referred clients to other source for this testing. Agency thought there might still be another local agency that could provide this and possibly the health department. Break 12:15p-12:25p In lieu of a break Committee members held brief discussion where it had additional questions that require follow-up. Members would like clarification on the following questions: • Why only the two STI tests and not Syphilis and HIV/AIDS? • Is this agency the only free STI testing in the community? • Would the funds be used to print materials • Can there be a requirement that if testing funded that the testing be more comprehensive for other common STD's TAB Applicant Organization Guest(s) Presentation Time 7 Denton Conununity Health Clinic(1) Alice Nhsciarelli PhD 12:30p-12:40p Came Breedlove The agency was provided with five minutes to present information to support its funding request. Denton Community Health Clinic's first presentation was for the behavioral health program. Agency started with a statistic that 45% of suicides visited their primary care physician less than 30 days before suicide. Agency explained the importance of comprehensive care that includes social emotional care. Agency shared 2 case studies. The emphasis on the behavioral health care is a comprehensive combination of health, medication, case management, counseling with the goal of reducing hospitalization and increasing out patient, client centered care. Agency explained it added interns to do more health care surveys and screenings. Agency shared that it also does outreach at Our Daily Bread. The goal is to psychosocial integrated care. HSAC members took five minutes to ask the agency questions. Agency was asked to speak about the relationship of behavioral health concerns and homelessness. Agency affirmed this relationship and explained it would share more about homelessness services in the next presentation. Agency was asked to share how behavioral health was assessed in a medical visit. Agency provided more information about assessments. Agency was asked about the application pages 6 and 8 where Part 4. and Part 6. have information from another application submitted. Agency was asked to provide updates to those pages. Agency transitioned into the next presentation. Denton Community Health Clinic's second presentation was for the Care Coordination Program. Agency opened with a case study. Agency discussed homelessness and the need for homeless to have integrated care. The agency described the program's partnerships, importance of relationships built meeting patients were they are. The agency highlighted a key element to advocate for patients by working with landlords to build trust and reduce barriers to housing. With the core philosophy that Housing = Health Care. Care Coordination reduces barriers to housing or prevents evictions, assist with employment, housing subsidies, legal services, legal documents and more because health care management is more lasting if the patient has housing stability. Agency concluded with a case study. HSAC members took five minutes to ask the agency questions. No additional questions were asked. Denton Community Health Clinic's final presentation was for the Diabetic Eye Exams. Agency explained that the eye exams are integral to diabetic care. Currently the agency has 33 patients who have received care from a specialized ophthalmologist. The retinal needs require specialized care that is provided by the ophthalmologist. Agency shared that the request is lower because they are able to leverage new funding source. Agency shared an additional benefit in their integrated care approach is that DCHC negotiates pricing and advocates for clients care needs with other providers, HSAC members took five minutes to ask the agency questions. Committee asked of the specialized ophthalmologist was needed, if the issue could be detected in a regular eye exam. Agency explained that it could but not all options would be available. The ophthalmologist for diabetic patients results in faster intervention. Agency then shared a case study. VII.
