AN oRDrNANCE ACCEPTING SEALED PROPOSALS AND AWA IN A CONTACT
FOR EMPLOYEE LONG AND SHORT TERM DISABILITY INSURANCE, PROVIDING
FOR THE EXPENDITURE OF FUNDS THEREFORE, AND PROVIDING AN EFFECTIVE
DATE (RFSP 2599- EMPLOYEE LONG AND SHORT TERM DISABILITY INSURANCE
AWARDED TO UNUM LIFE INSURANCE COMPANY iN THE ESTIMATED ANNUAL
COST OF $174,282)
WHEREAS, the C~ty has sol,c~ted, received and evaluated competitive sealed proposals
for the purchase of necessary materials, eqmpment, supphes or services ~n accordance w~th the
procedures of STATE law and C~ty ordmances, and
WHEREAS, the C~ty Manager or a designated employee has reviewed and recommended
that the here~n descnbed proposals are the lowest responsible proposals for the matenals,
eqmpment, supphes or services as shown ~n the "RESP Proposals" submitted therefore and that
they meet the evaluatmn cntena set forth ~n the Request for Sealed Proposals (the "EvaluaUon
Cntena"), and
WHEREAS, the C~ty Council has prowded ~n the C~ty Budget for the appropnaUon of
funds to be used for the purchase of the materials, eqmpment, supphes or services approved and
accepted herein, NOW, THEREFORE,
THE COUNCIL OF THE CITY OF DENTON HEREBY ORDAINS
SECTION I That the ~tems m the following numbered request for proposals for
materials, eqmpment, supphes, or servmes, shown ~n the "RFSP Proposals" on file m the office
of the Purchasing Agent, are hereby accepted and approved as being the lowest responmble
proposal meeting the Evaluatmn Cntena for such ~tems

SECTION II That by the acceptance and approval of the above numbered ~tems of the
subrmtted proposals, the City accepts the offer of the persons submm~ng the proposals for such
~tems and agrees to purchase the matenals, eqmpment, supphes or services m accordance w~th
the terms, specfficaUons, standards, quant~tles and for the specffied sums contmned ~n the RFSP
Proposal Invitations, RFSP Proposals, and related documents
SECTION III That should the City and person submxttlng approved and accepted ~tems
and of the submitted proposals wmh to enter xnto a formal written agreement as a result of the
acceptance, approval, and awarding of the proposals, the City Manager or hm demgnated
representaUve m hereby authorized to execute the written contract, provxded that the written
contract is in accordance with the terms, cond~txons, specfficatxons, standards, quanUtxes and
spemfied sums contmned ~n the RFSP Proposal and related documents herein approved and
accepted
SECTION IV That the C~ty Manager ~s hereby authorized to execute the Letter
Agreement, attached hereto and made a part hereof for all purposes, and contract wxth UNUM
Lxfe Insurance Company for RFSP 2599
SECTION V That by the acceptance and approval of the above numbered items of the
submitted proposals, the C~ty Council hereby authorizes the expendxture of funds therefor m the
amount and m accordance wxth the approved proposals or pursuant to a written contract made
pursuant thereto as authorized hereto
SECTION VI That thxs ordinance shall become effective ~mmed~ately upon xts passage
and approval
